Membership

Membership of the society shall be open to all Registered Medical Practitioners of Modern Medicine who practice Ophthalmology as a specialty or are interested in Ophthalmology and whose qualifications are satisfactory to the Managing Committee.

Terms And Conditions:

Admission of Member: Any person may be made Life member of the Society who agrees to abide by the Rules and Regulations of the Society. Such person shall make a written application to the Secretary recommended by two Life members of the Society who have personal knowledge of the applicant. The application form shall then be submitted to the Managing Committee for approval and admission. 

Subscription: Life Membership subscription shall be Rs 2500/- or as revised by the managing committee from time to time. 

Termination: Membership of the Society shall be terminated on the following grounds:

In case of resignation.

On death.

On conviction for a criminal offense involving moral turpitude or his/her incapacity to act as such for any reason whatsoever.

His/her conduct being found after due inquiry, harmful to the Society and or against its aims and objectives, provided that a written notice will be given to him/her and an opportunity afforded for being heard by the Managing Committee before his/her termination.

Renewal of Membership: A member who had tendered his/her resignation may be re-enrolled as a member at his/her request. 

Application form: Those who are interested in becoming members can take a printout of the application form and send it to the Secretary.

PLEASE SEND THE COMPLETED FORM ALONG WITH:

1. Two stamp size photographs
2. Photocopy of Medical Council Registration Certificate
3. Specimen signature on a plain paper to be enclosed with the form
4. Life Membership Fee is Rs 2,500/ (Rupees Two Thousand Five Hundred Only).
Deposit the amount in any Vijaya Bank Branch into KARNATAKA OPHTHALMIC SOCIETY A/C No 105501011001769 and send the ORIGINAL CHALLAN to the Secretary along with the completed Application Form. (AT PAR Cheque is also acceptable)
